Whoever YOU are I KNOW you have the strength to accomplish your goal, Write down what you need to do everyday and DO IT. Just start fucking doing it no matter how hard it is or how annoying/boring whatever it may be. Once you start doing this you'll begin to ENGRAVE this habit into your nervous system!!!
A Personal Trainer could be very beneficial creating that habit.
